How clients sign forms

How a form gets to a client, the signing link, and where signed forms are stored.

Once your forms are built, Lumè handles getting the right ones to each client. This article covers how a form reaches a client, how they sign it, and where the signed copy ends up.

How a form gets assigned

You don't attach forms by hand. When you book an appointment, Lumè automatically assigns the forms that apply — a first-time client's intake, and the consent forms for the services on that visit. You'll see any forms still needing a signature listed right on the appointment, and on the client's Documents tab.

How a client signs

Every assigned form has its own private link that opens a clean signing page — no account, no password. You can:

  • Send the client the link ahead of their visit so they arrive already done, or
  • Open it on a tablet at the front desk and hand it over for them to sign in person.

On the signing page the client fills in the fields and signs on screen with their finger or a mouse. When they submit, the page confirms it's signed. For privacy, that confirmation doesn't show the answers back on the public link — the completed form is saved to your records, not left readable on a link that might be forwarded.

Where signed forms live

A completed form is stored on the client's record, under the Documents tab, where your team can open and review it. Each signature is captured with the date and time and a full audit trail, so a signed consent stands up to scrutiny. The template is also snapshotted at signing, so editing a form later never rewrites what someone already signed.

If a form is ever voided, its old link stops working and shows a short notice telling the client to contact the spa for a new one.
