Building forms
Create intake and per-service consent forms with the form builder.
Lumè lets you build your own client forms — the intake paperwork a new client fills out, and the consent forms a treatment requires — instead of working around a fixed set of templates. Manage them under Forms in the menu, on the Client forms page. (The same area's EMR templates page is for clinical treatment records.)
Forms fall into two kinds:
- Intake forms — general paperwork, typically signed once when a client first comes in.
- Consent forms — tied to specific treatments, so the right consent is requested for the right service, at the right visit.
Creating a form
Select + New form and give it a name and a short description. Then choose:
- Type — intake or consent.
- How often it's needed — once ever, every visit, once a year, or whenever a particular service is booked. This is what lets a consent form re-collect a signature each time, while an intake form is asked for only once.
- Whether it's active — turn a form off to retire it without deleting the records already signed against it.
Adding fields
Build the form by adding fields with + Add field. Each field has a label, can be marked required, and can carry help text to guide the client. The field types available:
- Short text and long text
- Checkbox — for single agree/acknowledge boxes
- Multiple choice and dropdown — for a set of options you define
- Date
- Signature — the field the client signs
Connecting a consent form to services
For a consent form, choose which services should trigger it. From then on, when a client books one of those services, Lumè knows to collect that consent for the visit — you don't have to remember to attach it by hand.
